Dört Farklı Lise Türünde Ergenlerdeki Obsesif Kompulsif Bozukluk Yaygınlığı

Amaç: Dört farklı lise türünde ergenlerdeki obsesif kompulsif bozukluğun yaygınlığını saptamak amaçlanmıştır. Bununla birlikte subklinik OCD yaygınlığı saptamak, OKB’si olan ve olmayan öğrencilerin sosyodemografik verüerini kıyaslamak, OKB’si olan öğrencilerin başlangıç yaşım, obsesyon ve kompul-siyonlarm şiddet ve tiplerini saptamak amaçlanmaktadır.
Yöntem: Bu çahşma grubu dört farklı okul türünden 361 lise birinci sımf öğrencisinden oluşmaktadır. Yaş aralığı 14-17 arasındadır. Psikiyatrik değerlendirmeleri yüz yüze görüşme ile SCID 0KB modülü ve Y-BOCS kullanılmıştır.
Bulgular: Şimdiki ve subklinik 0KB yaygınlığı sırayla % 3.9 ve % 3 olarak saptanmıştır. Sağhk meslek lisesi öğrencileri arasındaki yaygınlık % 5.6, Anadolu Lisesi öğrencileri arasında % 4.6, Ticaret Meslek Lisesi öğrencileri arasında % 4.1, Devlet Lisesi öğrencileri arasında % 1.1 olarak saptanmıştır. Okullar arasında 0KB yaygınlığı açısından bir farklılık saptanmamıştır. 0KB başlangıç yaşı ortalama 12.9 yaş (8-14) olarak saptanmıştır. 0KB saptanan öğrencilerden % 14.3’ü ve 0KB saptanmayan öğrencilerin % 8.4’ünün psikiyatrist görüşmesi yaptığı saptandı ve bu oranlar benzerdi. OKB’li öğrencilerde ailenin birinci çocuğu olmama ve boşanmış ailenin çocuğu olma anlamlı olarak yüksekti. Kirlenme ve saldırganlık obsesyonları ve kontrol etme, sayma ve temizleme kompulsiyonları en yaygın obsesyon ve kompulsiyonlar idi. 0KB saptanan öğrencilerde Y-BOCS puam ortalam 13.1 ±5.2 olarak saptanmıştır. Sonuç: Öğrencilerdeki 0KB yaygınlığı okul tipleri açısından benzerdi. Ergen populasyonundaki 0KB şiddeti klinik örnekleme göre daha hafif görünmektedir. OKB’nin erken saptanması, daha iyi bir işlevsellik, daha az sıkıntı ve daha hafif bir klinik gidişe katkı sağlayabilir.
The Prevalence of Obsessive Compulsive Disorder in the Adolescents to Four Different Type of High Schools
Objective: We aimed to determine the prevalence of Obsessive Compulsive Disorder (OCD) in the
adolescent population to four different type of high schools. Determining the prevalence of subclinical
OCD, comparing sociodemographic data obtained from individuals with and without OCD and
determining the age at onset, severity and types of obsessions and compulsions among individuals
with OCD was aimed as well.Method: This study group comprises 361 first grade students chosen randomly from four different type of high schools. Ages of the subjects are 14-17 years. OCD module of the SCID and Y-BOCS were used to conduct a face to face interview for psychiatric evaluation.Results: We found that the prevalence of current and subclinical OCD were, 3.9 % and 3 %respectively. These rates were found that prevalences were 5.6 % among students at the vocational high school (medical), 4.6 % among students at the state preparatory school, 4.1 % among students at the vocational high school (commerce) and 1.1 % among students at the public high school. The differences in prevalence in terms of the type school the subjects were attending were statistically insignificant.Mean age at onset of OCD was 12.9 years (8-14 years). 8.4 % of students without OCD
and 14.3 % of students with OCD had previously seen a psychiatrist that it was similar. Not being the
first child in the family was significantly more common among students with OCD. Students with OCD
were more likely to have divorced parents. Contamination and agressive obsessions; checking,
counting and cleaning obsessions, were the most common. Mean Yale Brown Obsessive Compulsive
Scale total score was 13.1±5.2 in OCD patients.Conclusion: The prevalence of students with OCD in terms of the different type of schools were similar. The severity of OCD in adolescent population samples seems to less than that in clinical samples. Early recognition of OCD, may provide for a better functionality, less distress and a milder clinical course.
