Klasik nöroleptiklerle tedaviye dirençli psikozlarda sulpirid'in etkinliği ve emniyeti

Klasik nöroleptikler (haloperidol, klorpromazin, tiyoridazin, pimozid, trifluperazin, flufenazin v.b.) ile 6 haftalık tedaviye cevap vermeyen psikozlarda sulpirid'i n etkinliğini ve güvenirliğini belirlemek amacıyla bu çalışma yapılmıştır.
DSM-IV tanı kriterlerine göre şizofreni, şizofreniform bozukluk ve atipik psikoz tanısı konan ve klasik nöroleptiklerle tedaviye dirençli 60 yatan erkek hastaya ortalama 1000(400-1600)mg/gün dozda sulpirid verildi. Tedavi öncesi ve/veya tedavi süresince hastalara Global Klinik İzlenimi (CGI) ölçeği, UKU Yan Etki Derecelendirme Skalası, Kısa Psikiyatrik Değerlendirme Ölçeği (BPRS), Pozitif ve Negatif Semptomları Değerlendirme Ölçekleri (PSDÖ,NSDÖ), Cheuinard Ekstrapiramidal Semptom Derecelendirme Ölçeği (ESDÖ)uygulandı ve hastaların tedavi öncesi ve sonrası tam kan, formül lökosit, AST, ALT, alkalen fosfataz, üre, kreatinin değerlerine bakıldı.
Sulpirid'i n klasik nöroleptiklerle tedaviye dirençli DSM-IV tanı kriterlerine göre şizofreni, şizofreniform bozukluk ve atipik psikoz tanısı konan olgulardan %40 (n=24)'i nda orta ve iyi derecede etkili olduğu; yan etkiler açısından da iyi tolere edildiği bulundu.
Efficacy and safety of sulpiride in treatment-resistant psychosis
The purpose of this research was to evaluate the efficacy and safety of sulpiride in psychosis who were treatment-resistant to the classical neuroleptics such as haloperidol, thioridazine, pimozide, trifluperazine, flufenazin and chlorpromazine.
Sulpiride was administered to 60 male inpatients who were treatment- resistant to the classical neuroleptics such as haloperidol, thioridazine, pimozide, trifluperazine, flufenazin and chlorpromazine and diagnosed as schizophrenia, schizophreniform disorder,atypical psychosis according to DSM-IV criteria. The average dose was 1000 (400-1600)mg/day. Pre- and post-treatment evaluations of the subjects were carried out by using Clinical Global Impression (CGI), UKU Side-Effects Check List, Brief Psychiatric Rating Scale (BPRS), Andreasan's Scale for the Assessment of Negative Symptoms (SANS), Andreasan's Scale for the Assessment of Positive Symptoms (SAPS), Cheuinard's Scale for the Assessment of Extrapyramidal Symptoms. Moreover, for each subject, pre- and post-treatment blood analysis was conducted, and formula leucocyte, SGOT, SGPT, alkaline phosphatase and BUN levels were screened.
Our findings demonstrated that sulpiride brought improvement for 40% (n=24) of the cases, who did not respond to the previous drug treatment with classical neuroleptics, and it's side effects were well tolerated.
