Sülpirid, seçici olarak merkezi ve periferik dopamin D2 ve D3 reseptörlerine bağlanan, yapısal olarak standart antipsikotiklerden farklı olan bir benzamid türevidir. Klasik antipsikotiklerden farklı olarak yüksek dozlarda yatıştırıcı ve düşük dozlarda uyarıcı etki gösteren bir klinik etki yelpazesine sahiptir. Ketlenmeyi engelleyen etkisi nedeniyle otistik ve affektif belirtileri olan akut ve kronik şizofreni tedavisinde kullanılmaktadır.

Bu yazıda sülpirid tedavisi sonucu manik atak gelişen negatif belirtileri baskın olan kronik şizofreni tanısı almış 2 olgu sunulmuştur.

Sulpiride is a benzamide derivative, structurally distinct from other standart antipsychotics, which binds selectively to central and peripheral dopamine D2 and D3 receptors. Sulpiride has a spectrum of clinical activity which differs from that of conventional antipsychotics, with sedative effects predominating at higher doses and activating effects at lower doses. As a disinhibiting antipsychotic, sulpiride is indicated for the treatment of acute and chronic schizophrenia with prominent autistic and affective symptoms.

We describe here two cases with a diagnosis of chronic schizophrenia with predominating negative symptoms who demonstrates manic symptoms after sulpiride treatment.
