Parkinson Hastalığı?nda Depresyon için Sertralin ve Venlafaksin Tedavisinin Karşılaştırılması

Amaç: Parkinson Hastalığı'nda depresyon saptanan hastaların klinik özelliklerini incelemek, aynı zamanda farklı etki mekanizmaları olan antidepresan tedavi seçenekleri kullanarak bunların tedavi etkinliği, yan etkileri ile bilişsel fonksiyonlar ve Parkinsona hastalığına özgü motor bulgular üzerine etkisini değerlendirmek amaçlanmıştır.
Yöntemler: Örneklem grubu; 2006-2008 tarihleri arasında Nöroloji ve Psikiyatri A.D. poliklinikleri tarafından ayaktan izlenen Parkinson hastalarından oluşturulmuştur. DSM- IV tanı kriterlerine göre depresif bozukluk tanısı alan olgular çalışmaya alınmıştır. Hastalara Hamilton Depresyon Derecelendirme Ölçeği, Standardize Mini Mental Test (SMMT), UKU Yan Etki Değerlendirme Ölçeği ve Yapılandırılmış Parkinson Hastalığı Derecelendirme Ölçeği (UPDRS) testleri uygulanmıştır. Rastgele yöntemle seçilen olgulara sertralin ve venlafaksin başlanmıştır. 3, 6, 9 haftalarda testler yinelenerek sonuçları değerlendirilmiştir.
Bulgular: Yüz bir Parkinson hastası ile görüşülmüş, 35'inde (%34.6) depresyon saptanmıştır. Dokuzuncu haftada yapılan değerlendirmede sertralin tedavisi verilen 16 olgudan 14'ü (%87), venlafaksin tedavisi verilen 16 olgudan 12'si (%75) tedaviye cevap vermiştir. Arada anlamlı fark yoktur. SMMT skorları değerlendirildiğinde, sertralinin yönelim, dikkat, hafıza üzerine anlamlı olumlu etkisi görülürken, venlafaksinde görülmemiştir. Her iki ilaçta belirtilerde kötüleşme yapmamış ve antiparkinson tedavi üzerinde olumsuz etki oluşturmamıştır.
Sonuç: Parkinson hastalığında depresyon tedavisinde sertralin ve venlafaksinin etkin olduğu ve antiparkinson tedaviyi olumsuz etkilemediği görülmüştür. Tedaviye cevap açısından iki molekül arasında fark bulunmazken bilişsel fonksiyonlar üzerine sertralinin olumlu etkisinin daha belirgin ve yan etkilerin de daha az olduğu belirlenmiştir.
Comparison of Sertraline and Venlafaxine Treatments for Depression in Parkinson?s Disease
Objective: To investigate the clinical features of Parkinson's disease (PD) patients with depression, and consequently, by using anti-depressant agents with different mechanism of action, to evaluate their adverse effects and the effects on Parkinson-specific motor findings as well as cognitive functions.
Methods: The study group comprised patients with established PD diagnosis and followed up by Neurology and Psychiatry outpatient clinic between 2006 and 2008. The patients diagnosed with depressive disorder according to criteria specified in the DSM-IV were enrolled into the study. The Hamilton Depression Scale, Standardized Mini-Mental State Examination (SMMSE), UKU Side Effect Rating Scale and the Unified Parkinson's Disease Rating Scale (UPDRS) were applied. The patients were randomly assigned to treatment with sertraline or venlafaxine. All tests were repeated at 3, 6 and 9 weeks.
Results: A total of 101 patients with PD were evaluated, and depression was d etected in 35 (34.6%) of them. 14 out of 16 patients (87%) receiving sertraline and 12 out of 16 patients (75%) receiving venlafaxine responded to the treatment and no statistically significant difference was observed between the treatment groups. According to the SMMSE scores, there were significant favorable effects on orientation, attention and memory with sertraline, in contrast with venlafaxine. Both drugs did not show negative effect on anti-Parkinson treatment.
Conclusion: Sertraline and venlafaxine were found to be effective in the treatment of depression in patients with PD and they do not negatively affect the anti-Parkinson treatment. Furthermore, there was no difference between the two agents with respect to the treatment response, however, sertraline was found to have more pronounced positive effects on cognitive functions and less adverse effects than venlafaxine.
